Understanding Claims and EOBs

After you receive care, your provider may submit a claim to your health plan. Once processed, you may receive an Explanation of Benefits, or EOB.

An EOB shows:

• The amount billed

• The amount paid by your plan

• Discounts applied

• The amount you may owe

• The claim status

An EOB is not usually a bill. Compare it with any bill you receive from your provider.

Contact Member Services if you do not recognize a claim, disagree with the amount owed or believe the claim was processed incorrectly.
